Please find below the downloads for the summer enrollment forms (by level/age group). If you have any questions as to where your child should be placed, please do not hesitate to ask.
We ask for forms to be submitted, with fee, no later then May 31st, 2009. It is suggested to enroll early to ensure a spot in camp. An early enrollment discount of 10% will be offered for those enrolled by April 15th, 2009. If you have any problems downloading the forms please email us at mail@starsunlimiteddance.com.
